Fact-checking 'The Pitt' finale's 'wild' emergency C-Section

There have been many hugely memorable medical emergencies onHBO Max's "The Pitt,"the Emmy-winning phenomenon set during a single shift at a Pittsburgh emergency department. From the blood-soaked floors of the hospital during the response to a mass shooting, to disgusting moments of urination and defecation to the realities of hospice death, "The Pitt" doesn't shy away from the good, the heartbreaking and the gross parts of modern medicine. And the Season 2 finale (now streaming) has given us a new medical case for the TV hospital history books: An emergency C-section performed by Dr. Robby (Noah Wyle) and Dr. Abbott (Shawn Hatosy) right there in the ER.

It's a big scene, and not just because the episode graphically shows Robby and Abbott cutting into a pregant abdomen, pulling aside muscle and fascia and organs. It's tense, viscerally and emotionally, for all the characters involved. And it was a heck of a thing to pull off, according to Dr. Joe Sachs, a producer and medical advisor on "The Pitt."

"We needed to come up with something for the season finale that was rather dramatic," he said in a recent interview with USA TODAY.

Mission accomplished for "The Pitt" crew. Here's how they pulled off that gory emergency surgery, why they put it in the show and whether it really is what would happen if a pregnant woman had these complications.

How 'The Pitt' made that graphic C-Section look so real

In the episode, 15 hours into what is supposed to be Dr. Robby's final shift at Pittsburgh Trauma Medical Center before taking a sabbatical, a pregnant woman is brought in to the hospital. Complaining of a raging headache and with ankles so swollen poking them leaves an indent, she's rushed into the trauma room with Dr. Abbott (Shawn Hatosy), the attending physician on the night shift crew. The doctors and nurses quickly discover that the patient (Nicole Wolf) has opted for what she calls a "wild pregnancy," meaning she has not pursued prenatal care with an OBGYN or a midwife. That also means no one has diagnosed two life-threatening pregnancy complications she experienced that led to her ER visit: HELLP Syndrome and Preeclampsia. Her condition rapidly deteriorates to the point where Abbott and Robby are forced to do a C-Section in the ER, with seconds to spare to save her life and the life of the baby.

"It's at least two months of preparation," just for that one scene, says Sachs. "The [actor's] body is laser scanned and we create an entire silicone replica of the person's body. ... The silicone prosthetic piece will go on the upper chest and be blended seamlessly. So while we're doing the scene, you can have the real actor's head and arms, so it looks very real. Then, the special effects department is building a special gurney so that the actor's body can submarine under the bed."

Then, the actors are able to work on the prosthetic piece and the series shows them "cutting into the uterus, a flow of amniotic fluid, bringing the silicone baby out, floppy, and a real-looking umbilical cord that we could cut."

Most of that was done with practical effects, but they had to use CGI for one important element in order to keep the look right when the fake baby was removed. "When you pull the baby out, the (prosthetic) belly kind of goes down by 50%. And, you know, we didn't want to spend thousands of dollars building a second one."

In a case like the one "The Pitt" presents, things would happen as fast and furious as they do in the episode, says Dr. Dillon Knight, OB/GYN at Northwell’s Lenox Hill Hospital and Women’s Primary and Specialty Care at Astoria, both in New York City.

"In a true emergency, the cesarean is performed at the bedside wherever the patient arrested, not in an operating room," she says. "The goal is for the fetus to be delivered as quickly as possible and in most cases the time from skin incision to delivery is less than 60 seconds. The room would be chaotic with a large, multidisciplinary team working together to save the patient and her child. There would be ongoing CPR, multiple team members performing chest compressions, managing the airway, administering medications, and someone performing the surgery simultaneously. ... When performing a cesarean delivery outside of an operating room, there is no sterile field preparation or surgical draping; antiseptic solution may simply be poured on the abdomen."

Eagle-eyed parents who have been involved in c-sections for the birth of their own children might have noticed that Robby does a large vertical incision down the pregnant belly, not the horizontal lower abdomen incision that is most common in standard practice today. That's how things would be done in true emergency, Knight says. "In resuscitative cesarean delivery during maternal cardiac arrest, guidelines specifically recommend a vertical midline skin incision because it's fastest and provides options for further exploratory surgery if needed."

Do people really choose to have 'wild pregnancies'? Is it safe?

"The genesis of the story was an article [titled] 'Influencers made millions pushing wild births. Now the Free Birth Society is linked to baby deaths around the world,'" Sachs says, referencing aninvestigation published in The Guardian in 2025.

When Sachs discusses "wild pregnancy" and "free birth," he's not talking about women who choose to give birth at birthing centers or at home with a qualified midwife. He's talking about those who forgo any type of prenatal care at all, and choose to have only friends and family present during labor.

"Home births are fine. Midwives are excellent. But to do a free birth is extremely risky," Sachs says. Knight, the OBGYN, agrees.

"The risks are substantial and well-documented" when women forgo prenatal care, she says. Risks include "undetected and untreated conditions like preeclampsia, gestational diabetes, infections, including syphilis and HIV," for the mother, and "increased risk of stillbirth, respiratory distress, sepsis, and long-term developmental problems" for the baby.

Why does the finale C-Section matter to 'The Pitt' and Dr. Robby?

Wyle's Dr. Robby has been struggling with his mental health for the entire 15-hour shift that makes up Season 2 of "The Pitt." Over the 15 episodes the character has made troubling statements to his colleagues that hint he might be having suicidal thoughts. The producers wanted him to reckon with life and death in the finale, with a focus on life's beginnings.

"Robby in an existential crisis," Sachs says, of the character in the finale. He's "at the end of the shift, exhausted, possibly suicidal, yet being pulled in with Abbott, the night shift doctor, to do a crazy, heroic procedure, and being able to put that aside and focus on the excellence of his skill, his education, his experience to do something miraculous, even in the mental state that he's in. So that's the dramatic need of the story."

The "wild pregnancy" storyline is what allowed the emergency to be so intense and the need for Robby's skills to be so great: Preeclampsia affects 5-8% of pregnancies and HELLP affects about 10-20% of women with preeclampsia. Both are often diagnosed much earlier in pregnancy and can be treated in a variety of ways.

"We have to do character development in 15 hours, in one day," Sachs says.

"How much do you develop and change in 15 hours? How much do your friends develop and change?"

Singer D4vd arrested in the murder of teen months after she was found dead in trunk of his Tesla

The singer D4vd was arrested on suspicion of murder in connection with the death of a teenage girl whosedecomposed body was found in the trunk of his abandoned Tesla last year, authorities in Los Angeles said Thursday.

D4vd, 21, the “Romantic Homicide” singer whose real name is David Anthony Burke, was being held without bail, the Los Angeles Police Department said in a statement.

The case will be presented to the Los Angeles County District Attorney's Office on Monday, police said. The prosecutor's office said Thursday that its major crimes division will review the case.

Other California cases

Attorneys for the entertainer said they were prepared to "vigorously defend David’s innocence.”

“Let us be clear — the actual evidence in this case will show that David Burke did not murder Celeste Rivas Hernandez and he was not the cause of her death," attorneys Blair Berk, Marilyn Bednarski and Regina Peter said in a statement Thursday night after his arrest. "There has been no indictment returned by any grand jury in this case and no criminal complaint filed. David has only been detained under suspicion.”

Burke was arrested shortly after 4:30 p.m. at a house on Marmont Avenue in the Hollywood Hills, LAPD Capt. Scot Williams said.

"We did the best we can to keep tabs on him, but once we developed probable cause to arrest him for murder, then we were on him pretty diligently,” Williamstold the California Postin a statement he confirmed to NBC News.

On Sept. 8, police found the remains of Celeste Rivas Hernandez, 14, in the trunk of a Tesla impounded at Hollywood Tow. Officers were called to the lot because of a foul odor coming from the vehicle.

Inside a black cadaver bag in the vehicle's trunk, officers found a decomposed head and torso, according to court filings associated with a grand jury investigation. A second bag contained dismembered body parts.

The filings say Burkewas the target of the investigation.

It isn't clear how Celeste died. The medical examiner’s office said last year that it couldn't release details about her cause and manner of death because of a security hold sought by police.

The agency didn’t immediately respond to a request for comment Thursday.

Celeste was reported missing from Lake Elsinore, southeast of Los Angeles, in 2024, according to the Riverside County Sheriff’s Office.

The Los Angeles County medical examiner said she was wearing black leggings, a tube top, a yellow metal chain bracelet and stud earrings when she was found. Law enforcement sources told NBC News that the remains weren’t intact. The medical examiner said it appeared she had been dead in the car for an extended period of time.

When news of the remains’ being found broke, Burke was on his Withered World Tour. A spokesperson said at the time that he was aware of what had happened and was “fully cooperating with the authorities.”

After a quieter start to the year spent writing and creating, things are ramping up quickly. Meghan Marklehas made the first solo stop of her andPrince Harry's trip to Australia.

On the afternoon of Tuesday, April 14 (local time), the Duchess of Sussex, 44, visited a Homeless Services for Women Centre in Melbourne, where she put on an apron and served frittata to the users of the center.

After serving several people, Meghan asked, “Is anyone else hungry?" appearing relaxed and smiling at the press and guests at the location, PA Media reported.

Meghan then sat down at a table and joined people eating food at the centre, telling them: “We landed here this morning so my jet lag hasn’t quite hit yet.”

The center is run by McAuley Community Services for Women, which supports women and children experiencing family violence, homelessness and related challenges.

Prince Harry, 41, and Meghan's Australia visit marks their first time back on the continent since their royal tour in 2018. The trip is privately funded, andthe couple's children,Prince Archie, 6, andPrincess Lilibet, 4, will not be joining them.

The couple made their first public appearance of the visit on Tuesday morning, fora visit to the Royal Children’s Hospital in Melbourne. Harry's mother,Princess Diana, had previously visited the Royal Children's Hospital on her royal tour of Australia in 1983.

Harry and Meghan were greeted and met by cheers from large crowds gathered along the walkways and balconies of the Royal Children’s Hospital in Melbourne on Tuesday.

Novalie Morris, 12, a patient at the hospital who met the couple, told PEOPLE at the event, “I gave Harry flowers and he said 'thank you' and he told me to 'keep on being brave.' It cheered me up a lot and I’ll keep thinking about that."

The youngest onlooker was six-week-old baby Millie who, after being released three days ago, returned to the hospital for a check-up. Her mother Zoe said it was a highlight for the family after a tough few weeks.

“One day, I’ll be able to tell her she met the prince," she told PEOPLE.

One 4-year-old patient, Lily, gave the couple a hand-drawn sign that said, “Welcome Harry and Meghan,” and presented a flower to the duchess, who hugged the little girl saying, “Oh my gosh, this is so sweet. I love it.”

Harry said, "Nice to meet you, Lily," and told her that the sign was "beautiful," asking, “How long did it take?”

During their trip, Prince Harry and Meghan are expected to attend a variety of public and private engagements,including Meghan headlining Her Best Life podcast's "girls weekend" retreatin Sydney and Harryserving as a keynote speaker on workplace mental healthat the InterEdge Psychosocial Safety Summit in Melbourne.

Their last trip to Australia came in October 2018, when they embarked on an official royal tour that included supporting Prince Harry's Invictus Games in Sydney. The coupleannounced Meghan's pregnancywith their first child just before the tour kicked off, and Prince Archie was born in May 2019.

That 16-day tour of Australia, Fiji, Tonga and New Zealand included stops in Sydney, Dubbo, Melbourne, Fraser Island and the lakeside town of Rotorua in New Zealand.

Brooke Hoganshared a heartbreaking message to her late fatherHulk.

Nearly nine months after theWWE legend's death, Brooke, 37, shared a black-and-white illustration of her father holding her in his arms onInstagramTuesday, April 14.

Over the image, Brooke wrote, "If my love could have saved you, you would have lived forever."

Hulk died after he suffered a heart attack at his home in Florida on July 24. Brooke, who had a complicated relationship with her father, broke her silence five days later with a lengthy tribute.

"My dad's blood runs through my veins. His eyes shine through my children," she began in the post, referring to her twins with former NHL star Steven Oleksy.

“When he left this earth, it felt like part of my spirit left with him. I felt it before the news even reached us,” Brooke, whoasked to be removed from her father's willafter his death, continued in her tribute. "... I know he’s at peace now, out of pain, and in a place as beautiful as he imagined. He used to speak about this moment with such wonder and hope. Like meeting God was the greatest championship he’d ever have."

She also said she felt it was "necessary to clear a few things up" and explained that it was not a single "fight" or one moment that caused their estrangement."It was a series of private phone calls no one will ever hear, know, or understand," she said.

"My father was confiding in me about issues weighing on his heart, both personal and business. I offered to be a life raft in whatever capacity he needed. I told him he had my support. I begged him to rest, to take care of himself."

Oleksy, 40, told PEOPLE shortly after Hulk's death that the WWE legend had "no interest" in meeting his grandkids despite Brooke being at Hulk's side "for every surgery leading up to the last two years."

Brooke chose not to attend Hulk's funeral,explaining on Instagram afterthat she didn't think he would want one.

"My father hated the morbidity of funerals. He didn't want one," she said. "And although I know people grieve in many ways - and I'm so grateful for all celebrations and events organized to honor him, as his daughter, I had to make my own decision to honor him the best and most genuine way I knew how...privately...the way that made me feel the closest to him."

Hulk's life and death is the focus of anew Netflix docuseries,Hulk Hogan: Real American, premiering April 22, which will include his final interview. Brooke does not appear in the docuseries.
